Application Procedures

 

1. Complete Application Packet, including Parent-School Contract, Medical, and Driver approval forms / or apply online.

2.Obtain required reference forms

 

3. Include copies of the following:

     a. Most recent report card and achievement test

     b. Transcript

     c. Immunization record

 

4. Send the completed packet to the Lifegate, 1052 Fairfield Ave., Eugene, Oregon, USA 97402

OR email to office@lifegatechristian.org

 

5.  Wait for your acceptance letter and I-20

(to process US study visa)
